Description
Arising near the cardiac apex on the diaphragmatic (inferior) surface, the middle cardiac vein runs superiorly in the posterior interventricular sulcus toward the coronary sulcus. Its course places it between the right and left ventricles on the inferior aspect of the heart, closely paralleling the posterior interventricular artery (posterior descending artery) and flanked by ventricular myocardium on either side. Proximally, the vessel typically drains into the coronary sinus near the crux, where the atrioventricular and interventricular grooves intersect. Blue venous coloration separates it cleanly from adjacent coronary arteries and the red ventricular surface. Recognition of the middle cardiac vein matters when you need reliable venous landmarks on the diaphragmatic surface and in the posterior interventricular groove.
