Description
Right anterolateral cardiac surface is presented with the small cardiac vein coursing along the acute margin of the heart, where it typically accompanies the right marginal artery and drains the free wall of the right ventricle. Proximally, its tributaries arise as venules from the anterior cardiac veins and the venous network over the right atrium, then converge toward the coronary sulcus (atrioventricular groove). Near the base, the small cardiac vein continues posteriorly to join the coronary sinus close to its right atrial opening, with the superior and inferior venae cavae, pulmonary trunk, ascending aorta, and pulmonary veins providing orientation at the superior aspect. This right-sided emphasis matters when you are teaching coronary venous drainage beyond the coronary sinus itself, since the small cardiac vein is the key surface landmark for venous return from the right ventricular margin and can vary in caliber or even be duplicated. It also frames procedural anatomy: cannulation of the coronary sinus for retrograde cardioplegia, cardiac resynchronization therapy leads, or electrophysiology mapping depends on recognizing venous ostia near the right atrium, and a prominent small cardiac vein can be mistaken for an arterial structure on operative inspection or angiography. Small vessel, big consequences.
