Description
Seen from the right side, the middle cardiac vein (vena cordis media) tracks in the posterior interventricular sulcus from the inferior aspect of the heart toward the crux, running alongside the posterior interventricular artery (posterior descending artery). Its course separates the right and left ventricles posteriorly, with the apex positioned inferiorly and to the left in standard anatomical orientation. Superiorly, the great vessels at the base are present, including the ascending aorta and pulmonary trunk with their proximal branches, while the fibrous pericardium is omitted to expose epicardial coronary vessels. This right-sided perspective matters because the middle cardiac vein is one of the most consistent venous companions of the posterior interventricular artery and provides a reliable surface landmark for the posterior interventricular groove during posterior cardiac exposure. The vein typically drains into the coronary sinus near the crux cordis, a junctional region surgeons and electrophysiologists reference when navigating the atrioventricular groove and planning coronary sinus cannulation. For teaching coronary circulation, it also clarifies the arterial-venous pairing patterns on the diaphragmatic surface, where anatomy can be harder to conceptualize from anterior-only diagrams.
