Large Great Cardiac Vein

Description

Prominent on the left anterolateral surface, the great cardiac vein courses in the anterior interventricular sulcus alongside the left anterior descending artery, then turns superiorly and leftward within the left atrioventricular (coronary) sulcus. From there it enlarges as it approaches the posterior aspect of the heart to continue into the coronary sinus, with smaller venules and tributaries converging from the anterior left ventricle and interventricular septum. Superiorly, the ascending aorta and aortic arch rise above the base, and the pulmonary trunk with its branches frames the arterial outflow while the epicardial vascular network spreads across both ventricles. This is the venous counterpart clinicians often forget until the coronary sinus becomes the target. Electrophysiology teams advance left ventricular pacing leads through the coronary sinus and its tributaries, and the great cardiac vein, at its transition into the coronary sinus, helps you understand where lead delivery can be limited by valves, tortuosity, or adjacent arterial structures. Venous anatomy also matters when interpreting coronary CT angiography or MR venography, where a prominent great cardiac vein can be mistaken for an arterial structure unless its course in the sulci and its drainage pattern are clear. Landmarks matter.
