Description
Anterior cardiac landmarks frame the superficial venous drainage: the great cardiac vein ascends in the anterior interventricular sulcus alongside the left anterior descending artery, then turns leftward in the atrioventricular (coronary) sulcus to continue as the coronary sinus on the posterior aspect. Rightward, the small cardiac vein tracks along the right margin and right atrioventricular groove toward the coronary sinus, while anterior cardiac veins cross the right ventricular free wall to empty directly into the right atrium. Tributary venules on the epicardial surface converge into these named channels, running superficial to the myocardium but closely tethered to the coronary arteries by shared sulci and connective tissue. Major inflow and outflow vessels, including the ascending aorta, pulmonary trunk, and superior and inferior venae cavae, provide orientation superiorly and posteriorly. Venous anatomy tends to get less attention than the coronaries until you need it. Electrophysiology and heart failure procedures routinely cannulate the coronary sinus for left ventricular lead placement in cardiac resynchronization therapy, and success hinges on understanding where the great cardiac vein becomes the sinus and how its ostium sits in the right atrium near the tricuspid annulus. Variants such as a prominent anterior cardiac venous system or small-caliber coronary sinus can complicate catheter navigation and increase the risk of dissection or perforation. Use this rendering for teaching the relationship of the coronary veins to the anterior interventricular and atrioventricular grooves in gross anatomy, cardiovascular physiology, and interventional cardiology modules, or to illustrate device therapy workflows in clinical texts covering coronary sinus cannulation and venography.
