Description
Running in the anterior interventricular sulcus, the anterior interventricular vein (great cardiac vein) courses inferiorly toward the cardiac apex alongside the anterior interventricular artery (left anterior descending, LAD). It lies on the anterior surface of the ventricles, typically between the right ventricle laterally and the left ventricle medially, then turns superiorly in the left atrioventricular (coronary) sulcus. Superiorly, the ascending aorta and proximal great vessels form the deep background, while smaller venules converge into the main vein across the epicardial fat. Clinically, this vein is the landmark venous companion to the LAD, a relationship used constantly when orienting to the anterior interventricular groove during coronary artery bypass grafting and when correlating anterior wall myocardial infarction territories on angiography or CT coronary imaging. Its continuity with the great cardiac vein and eventual drainage to the coronary sinus makes it relevant in electrophysiology and heart failure work, where coronary venous anatomy can constrain left ventricular lead placement for cardiac resynchronization therapy. Small vessel, big consequences.
