Description
Oblique left lateral anatomy of the heart brings the coronary sinus into clear relief along the posterior atrioventricular (coronary) sulcus, where it courses horizontally toward the right atrium. Superior to this venous channel, the left atrium sits posteriorly while the pulmonary trunk and ascending aorta rise from the base, and the left ventricle forms the inferolateral bulk of the cardiac silhouette. Blue venous tributaries on the diaphragmatic and posterior surfaces logically include the great cardiac vein turning into the coronary sinus near the left atrioventricular groove, with the middle cardiac vein approaching from the posterior interventricular sulcus. Red coronary arteries track nearby in the epicardial fat, reinforcing the artery-vein relationship on the surface. Coronary sinus anatomy matters because it is the dominant venous outflow of the myocardium and a consistent procedural landmark at the posteroinferior aspect of the heart. Cannulation of the coronary sinus under fluoroscopy or transesophageal echocardiography is routine for cardiac resynchronization therapy, where a left ventricular lead is advanced into a lateral or posterolateral cardiac vein. Lead placement fails most often when the ostium is difficult to engage, the valve of Thebesius is prominent, or the target venous branch is small or sharply angulated, all issues that become easier to teach when the coronary sinus is seen in correct spatial context against the left atrium and left ventricle.
