Description
Right lateral orientation emphasizes the right atrium and the atrioventricular (coronary) sulcus, where the coronary sinus courses transversely on the posterior aspect of the heart before turning toward its atrial opening. Along its path, the sinus receives the great cardiac vein as it continues from the anterior interventricular sulcus, and it is typically joined by the middle cardiac vein ascending in the posterior interventricular sulcus and the small cardiac vein tracking the right margin. Superior to this venous confluence, the pulmonary trunk and ascending aorta rise anteriorly from the ventricular outflow tracts, helping you keep anterior and superior relations straight. Coronary sinus anatomy matters most when you need dependable access to the cardiac venous system. Electrophysiology teams routinely cannulate the coronary sinus from the right atrium to place left ventricular leads for cardiac resynchronization therapy, map left-sided accessory pathways, or deliver retrograde cardioplegia in the operating room.
