Description

Arising from small venules along the anterior surface of the left ventricle, the anterior interventricular vein (great cardiac vein in its anterior course) ascends in the anterior interventricular sulcus toward the base of the heart. It tracks beside the anterior interventricular branch of the left coronary artery (left anterior descending artery), typically lying slightly to its left as both vessels run superiorly toward the coronary sulcus. Nearby external landmarks include the right and left auricles, the pulmonary trunk anteriorly at the base, and the ascending aorta curving superiorly and posteriorly. This left-sided anterolateral perspective clarifies the coronary venous drainage that converges into the coronary sinus, a relationship that can feel abstract on flat diagrams. It is a practical orientation for correlating angiographic and CT coronary anatomy: the anterior interventricular vein often parallels the LAD so closely that it becomes a useful surface guide, but it also creates a hazard during anterior interventricular groove dissection and during exposure of the LAD for coronary artery bypass grafting. Venous enlargement or displacement can also be appreciated in the setting of anterior wall myocardial infarction, where edema and epicardial fat planes obscure smaller tributaries. Use this asset in gross anatomy and cardiovascular block teaching when you want learners to distinguish coronary veins from coronary arteries on the intact heart, and to locate the anterior interventricular sulcus quickly.
