Location of the Coronary Sinus

Description

Arising on the posterior aspect of the heart and best appreciated from an inferior (diaphragmatic) perspective, the coronary sinus runs transversely in the posterior atrioventricular (coronary) sulcus between the left atrium superiorly and the left ventricle inferiorly. Its lumen lies just left of midline before curving medially to open into the right atrium, near the interatrial septum at the region of the inferior vena cava orifice. Superficial coronary veins converge toward it, with the great cardiac vein typically approaching from the left along the atrioventricular groove, while neighboring epicardial coronary arteries course in parallel but remain distinct in color and caliber. Spatial orientation is clear. This angle matters when you need to teach or plan procedures that depend on locating the coronary sinus ostium and its course along the posterior atrioventricular groove. Electrophysiology studies commonly place a catheter in the coronary sinus to record left atrial and left ventricular activation, and coronary sinus cannulation can be the limiting step in cardiac resynchronization therapy when placing a left ventricular pacing lead via a lateral venous branch. The inferior view also helps explain why posterior myocardial infarction or mitral annular surgery can complicate venous drainage and increase the risk of coronary sinus injury or thrombosis. Use this rendering in gross anatomy and cardiothoracic modules to reinforce the relationship of coronary venous return to the atria and atrioventricular junction, and in EP, device, or cardiac imaging teaching materials when correlating fluoroscopy, CT, or MR venography with surface anatomy.
