Description
Presented in a right lateral (right-side) perspective, the heart is oriented to highlight the inferior vein of the left ventricle as it courses over the diaphragmatic (inferior) surface of the left ventricular myocardium toward the posterior aspect of the atrioventricular groove. The left ventricle forms the dominant inferior contour, while the left atrium sits superior and posterior, partly obscured by the ventricular mass from this angle. Also visible are the proximal great vessels, including the ascending aorta and arch superiorly, with pulmonary vessels clustered anterior to the atrial region, and a superficial network of coronary vessels running in the interventricular and atrioventricular sulci. Emphasis on the inferior venous channel matters because left ventricular venous drainage is less stereotyped than arterial supply and becomes clinically relevant in coronary sinus–based procedures. Mapping the inferior vein of the left ventricle helps orient catheter trajectories during coronary sinus cannulation, placement of left ventricular leads for cardiac resynchronization therapy, and retrograde cardioplegia, where venous anatomy, ostial angles, and adjacent fat in the atrioventricular groove can complicate access. Small structure. Big consequence. In teaching, this view supports cardiovascular gross anatomy labs, cardiology fellowship lectures on coronary venous anatomy, and surgical atlases covering posterior and diaphragmatic surface landmarks used during mitral valve exposure or coronary bypass planning. It also fits radiology and interventional education when paired with CT coronary venography or venous phase angiography to reconcile surface vessels with imaging appearances.
