Description
Prominent on the anterior surface of the heart, the anterior cardiac veins course over the right ventricle, running superiorly toward the right atrium and lying superficial to the myocardium in the anterior interventricular and right atrioventricular (coronary) grooves. Several small venules converge into these veins along the sternocostal surface, with tributaries tracking near the course of the right coronary artery and its marginal branches. The right atrium sits superior and medial to the right ventricle, while the left ventricle forms the left border and apex, providing a clear lateral contrast for the predominantly right-sided venous drainage. Focus on these veins matters because they represent a direct venous return pathway to the right atrium that bypasses the coronary sinus, a point that often confuses trainees when they first map coronary venous anatomy. Electrophysiology and cardiothoracic teams care about this territory: right atrial cannulation, placement of right-sided pacing leads, and surgical manipulation along the right atrioventricular groove can injure small anterior cardiac veins, creating nuisance bleeding that is hard to control on a beating heart. Their relationship to the epicardial fat and the right coronary artery also makes this view a clean teaching reference for separating venous channels from arterial companions.
