Description
Posterior elbow anatomy is centered on the olecranon process of the ulna, where a fibrous posterior retinaculum spans as an aponeurotic expansion blending with the deep fascia over the triceps tendon. Proximally, the triceps brachii tendon converges toward its insertion on the olecranon, while distally the fascia continues into the posterior forearm compartment over the extensor muscle mass. Medially and laterally, the retinacular fibers lie superficial to the posterior aspects of the ulnar and radial margins of the joint line, forming a tensioning band across the back of the elbow. Superficial planes may also suggest the course of the ulnar nerve as it passes posterior to the medial epicondyle, a nearby but distinct landmark. Posterior retinacular tissue matters because it is one of the structures that can be tightened, thickened, or disrupted in posterior elbow trauma and postoperative scarring, with direct implications for terminal extension and the feel of the posterior soft tissue envelope. Orthopedic approaches that split or reflect triceps (for olecranon fracture fixation, distal humerus exposure, or elbow arthroplasty) often traverse this fascia, so understanding its continuity with the triceps expansion helps you predict where adhesions form and why posterior impingement can persist even when the articular surface appears congruent.
