Description
Medial exposure of the male elbow centers on the anterior retinaculum as a transverse fibrous band spanning the anterior aspect of the joint, positioned superficial to the brachialis and deep to the distal biceps brachii tendon as it approaches the radial tuberosity. Proximally, the distal humerus forms the roof of the cubital fossa, while distally the proximal ulna and radius frame the joint line, with the retinacular fibers crossing anterior to the radial head and neck. Medially, the common flexor-pronator mass arises from the medial epicondyle and runs inferiorly into the forearm flexor compartment; laterally, the brachioradialis and extensor compartment contour the forearm border. The relationship is tight. This medial perspective matters because the anterior retinaculum is one of the soft-tissue restraints that helps maintain congruence at the radiocapitellar articulation during forearm rotation, and it sits in a neighborhood where clinicians routinely work. A hook test for distal biceps rupture, anterior approaches to the radial head, and assessment of the bicipitoradial bursa all depend on understanding how the biceps tendon, lacertus fibrosus (bicipital aponeurosis), and adjacent retinacular tissue layer over the brachialis at the front of the elbow. Subtle tears or thickening can alter tendon excursion and contribute to pain with resisted supination.
