Deep Femoral Vein

Description

Anterior thigh venous anatomy is centered on the deep femoral vein (vena profunda femoris), running deep to sartorius and rectus femoris within the femoral triangle region and then descending posterolaterally alongside the profunda femoris artery. Perforating veins course proximally from the posterior and medial compartments, converging on the profunda system near the linea aspera before it joins the femoral vein (vena femoralis) inferior to the inguinal ligament. Medially, the great saphenous vein (vena saphena magna) tracks superficially toward the saphenofemoral junction, while the external iliac vessels and pelvic venous channels are suggested superiorly as the inflow pathway to the femoral vein. Venous mapping at this level matters when you need to distinguish superficial reflux from deep system disease and to orient yourself to the profunda femoris as the main outflow route for the deep thigh musculature. Deep venous thrombosis in the femoral or profunda system can present with thigh pain and swelling and carries a higher embolic risk than isolated calf thrombosis. For vascular access and regional procedures, the relationship of femoral vein medial to the femoral artery in the femoral triangle is a landmark, but the profunda confluence is a common source of anatomic variation and a potential pitfall during duplex ultrasound or groin surgery.
