Description

Posteriorly, the deep femoral vein (vena profunda femoris) ascends in the proximal thigh, running deep to the hamstring compartment and posterior to the femoral shaft before joining the femoral vein more anteriorly at the level of the femoral triangle. Perforating veins traverse the adductor magnus to connect the deep system to the posterior compartment, while the popliteal vein continues proximally from the popliteal fossa toward the adductor hiatus. Superficially, the great saphenous vein is mapped along the medial leg and thigh, with visible tributaries tracking toward the saphenofemoral junction. Relationships matter here because posterior thigh venous drainage is often taught as an “anterior” story, yet many clinically relevant channels lie deep and posterior before they communicate with the femoral vein. This view helps clarify where thrombus can propagate from calf and popliteal veins into the profunda femoris and then into the femoral vein, a pathway assessed in duplex ultrasound for suspected proximal deep vein thrombosis. It also aligns with posterior surgical corridors and trauma patterns where hematoma, compartment swelling, or penetrating injury can compromise deep perforators and the profunda femoris vessels.
