Description
Anteriorly, the femoral vein ascends in the proximal thigh just medial to the femoral artery, continuing superiorly beneath the inguinal ligament to become the external iliac vein in the pelvis. Superficial to it, the great saphenous vein tracks along the medial leg and thigh to pierce the fascia at the saphenous opening and join the femoral vein near the femoral triangle. Distally, paired venules and muscular tributaries converge toward the femoral vessel, while perforating veins communicate from the superficial system into the deep venous channel. Orientation is in standard anatomical position, with the pelvic venous connections shown superiorly and the lower limb drainage depicted inferiorly. Clinical work in the groin often starts with this anatomy. The relationship of the femoral vein to the artery at the femoral triangle and as it courses toward the adductor canal explains why ultrasound guided femoral venous cannulation can drift from a safe medial puncture to an arterial stick if the limb is rotated or the probe is angled. Deep vein thrombosis frequently involves the femoral or popliteal segments, and thrombus propagation proximally into the external iliac vein correlates with more extensive limb swelling and higher pulmonary embolism risk. Compression points and valve related flow direction make sense here. Simple but unforgiving.
