Dorsal Venous Arch

Description

Across the dorsum of the male foot, the dorsal venous arch sweeps transversely over the tarsal region, just superficial to the metatarsals and the dorsal interosseous spaces. Medially, tributaries from the hallux and medial dorsal digital veins converge toward the great saphenous vein as it ascends anterior to the medial malleolus, while laterally the arch continues toward the small saphenous vein passing posterior to the lateral malleolus. Dorsal metatarsal veins and dorsal digital veins are shown coursing proximodistally over the shafts of the metatarsals and phalanges, with the venous plexus draped across the dorsal aspects of the tarsal bones. Bony landmarks remain readable beneath the vessels, including phalanges, metatarsals, and the contours of the cuneiforms and cuboid. Surface venous anatomy on the dorsum of the foot matters because this is where clinicians often identify and cannulate peripheral veins when upper-extremity access fails. Placement looks straightforward, but valves, small caliber, and overlying extensor tendons can complicate catheter advancement and increase infiltration risk, so an accurate map of the dorsal venous arch and its venules is practical, not academic. Varicosities and chronic venous insufficiency also declare themselves here as dilated, tortuous dorsal veins, and post-traumatic or post-surgical edema can obscure the normal transverse course of the arch. Use this illustration in gross anatomy and vascular anatomy teaching to anchor discussions of superficial venous drainage into the great and small saphenous systems, and in nursing, paramedic, and anesthesia training materials focused on difficult IV access and vein selection on the foot. It also fits podiatry and orthopedic publications that need clean dorsal foot landmarks while still showing the superficial venous network in context with the metatarsals and tarsals.
