Description
Across the dorsum of the male foot, superficial venules converge into the dorsal venous arch running transversely at the level of the metatarsal bases, just distal to the ankle joint and overlying the extensor tendons. Medially, the arch continues proximally as the great saphenous vein as it passes anterior to the medial malleolus, while laterally it feeds the small saphenous vein coursing posterior to the lateral malleolus. Beneath the vascular layer, the phalanges, metatarsals, and tarsal bones provide clear bony landmarks for surface mapping of the dorsal venous network. Landmarks matter. This dorsal projection is the one clinicians use when they cannulate a dorsal metatarsal vein or assess venous filling in peripheral arterial disease, where dependent rubor and delayed capillary refill often coexist with collapsed superficial veins. The continuity of the dorsal venous arch with the saphenous system also explains how distal superficial thrombophlebitis can track proximally, and why saphenous vein harvesting for coronary or peripheral bypass has downstream consequences for venous drainage and edema. For teaching, pairing the visible superficial veins with the underlying skeletal anatomy helps anchor where perforators and deep venous companions lie, even when those deeper channels are not drawn.
