Description
Arising from the transverse process of L5 (often with slips from L4), the iliolumbar ligament spreads inferolaterally to the internal surface of the iliac crest, forming a broad, fan-like band at the posterior part of the iliac fossa. Posterior to it sit the sacrum and sacroiliac joint, while the ilium wraps laterally toward the acetabulum where the femoral head articulates. Superiorly, the lower lumbar vertebrae and intervertebral discs frame the lumbosacral junction; inferiorly, the pelvic ring continues toward the ischium and pubis. Clear depiction of the iliolumbar ligament matters because it is one of the principal passive stabilizers of the lumbosacral junction and an anatomic boundary for surgical and injection-based work around the posterior iliac crest. Tenderness at its iliac attachment is a recognized contributor to mechanical low back pain patterns that mimic sacroiliac joint dysfunction, and it is frequently discussed in the differential when SI joint provocation tests are positive but imaging is unrevealing. Orientation relative to the L5 transverse process is also a practical landmark during posterior approaches to the lumbosacral spine and when planning trajectories that risk the adjacent lumbar arteries and ventral rami.
