Description
Arising from the transverse process of the lowest lumbar vertebra, the iliolumbar ligament is shown coursing inferolaterally to anchor on the iliac crest and iliac fossa along the inner aspect of the pelvic wing. An anterior perspective also places the ligament superior to the sacroiliac joint and lateral to the lumbosacral junction, with the sacrum and pelvic brim providing clear bony context. Portions of L4 to L5, the sacral promontory, and the paired hip bones (ilium, ischium, pubis) frame the attachment sites. Orientation is unambiguous. That spatial relationship matters because the iliolumbar ligament is a primary restraint to lateral flexion and anterior shear at L5 to S1, and it is often implicated in mechanical low back pain patterns that localize to the posterior iliac crest and mimic sacroiliac joint pain. Palpation tenderness near the posterior superior iliac spine is common, but the ligament’s origin sits higher and more medial, and this view helps clarify why fluoroscopic or ultrasound guided injections must target the lumbosacral junction rather than the superficial sacroiliac sulcus.
