Description
Spanning the ventral aspect of the pelvis, the anterior sacroiliac ligament appears as multiple thin fibrous bands bridging the anterior surface of the sacrum to the adjacent iliac wing at the auricular region. The ligament lies deep to the iliacus muscle and anterior to the synovial portion of the sacroiliac joint, blending with the anterior joint capsule rather than forming a single discrete cord. Medially, the fibers approach the sacral ala and anterior sacral foramina, while laterally they spread over the iliac side of the joint, just inferior to the iliac fossa and superior to the pelvic brim. Anterior ligament anatomy matters because it is the least substantial component of the sacroiliac ligamentous complex, so pain generators and instability often relate more to the interosseous and posterior sacroiliac ligaments even when tenderness is felt anteriorly. This is a common point of confusion. For anterior surgical and interventional approaches, including anterior SI joint exposure during pelvic ring fixation or image guided SI joint injection, understanding what constitutes capsule versus true anterior sacroiliac ligament helps prevent overestimating the structural restraint provided on the ventral side. The nearby L5 ventral ramus and lumbosacral trunk, which cross the pelvic brim medial to the iliopsoas, frame the clinical relevance of staying on bone and respecting the anterior joint line.
