Description
Framed within a cranial section through the orbit, the globe sits centrally with the sclera continuous anteriorly into the cornea and posteriorly into the optic nerve. Four rectus muscles, superior, inferior, medial, and lateral rectus, course from the posterior orbit toward the anterior sclera, forming a muscular cone around the optic nerve. The superior oblique runs along the superior-medial orbital roof toward the trochlear region before angling posterior-lateral to the globe, while the inferior oblique originates anteriorly on the orbital floor and passes posterolaterally beneath the inferior rectus. Insertions fall anterior to the equator of the eyeball. Extraocular muscle anatomy is where ocular motility becomes testable at the bedside and explainable in the lab. The spatial relationship between the lateral rectus and the abducens nerve, and between the superior oblique and the trochlear nerve, underpins classic patterns of diplopia in cavernous sinus lesions and orbital apex syndromes; a weak lateral rectus gives impaired abduction, while superior oblique palsy produces vertical diplopia that worsens on downgaze. Strabismus surgery also depends on these insertions and paths, with recession and resection procedures planned around each rectus tendon’s arc of contact on the sclera. Small millimeters matter.
