Description
Arising from the posterior aspect of the external carotid artery, the occipital artery courses superiorly and posteriorly, passing deep to the posterior belly of the digastric and the mastoid process before climbing toward the occiput. Along its cervical segment it runs in close relationship to the hypoglossal nerve and internal jugular vein region, then tracks posterior to the auricle as it approaches the superior nuchal line. Muscular branches radiate into the deep posterior neck, supplying splenius capitis, longissimus capitis, and adjacent suboccipital musculature, while terminal scalp branches spread over the posterior scalp. A lateral head and neck perspective makes the artery’s posterior trajectory easy to follow. For surgeons, this vessel is more than a named branch, it is a frequent contributor to scalp perfusion and a reliable landmark in posterior cervical dissection. Ligation or injury can complicate procedures around the mastoid and upper neck, and the occipital artery is routinely considered in reconstructive planning for posterior scalp defects and in flap design. It also matters in pain medicine, since the greater occipital nerve typically crosses near the occipital artery at the superior nuchal line, a relationship relevant to occipital neuralgia evaluation and targeted injections. Small, but unforgiving. Use this asset in head and neck anatomy teaching blocks, surgical atlas figures for posterior neck approaches, or clinical education materials discussing occipital scalp vascular supply, temporal bone surgery landmarks, or occipital nerve blocks.
