Description
Opening from the left ventricular outflow tract into the ascending aorta, the aortic valve appears with three semilunar cusps retracted toward the sinuses of Valsalva, apposed to the aortic wall to create a central, round or slightly triangular orifice. Superior to the cusps lies the aortic root and proximal ascending aorta, while inferiorly the valve annulus is continuous with the fibrous skeleton of the heart and the interventricular septum. The right and left coronary cusps sit anteriorly and leftward relative to the noncoronary cusp, and the coronary ostia occupy the corresponding sinuses just superior to the cusp attachments. Commissures define the cusp junctions at the sinotubular junction. Flow is antegrade. This open-valve configuration matters because it frames the left ventricular outflow tract anatomy clinicians interrogate when grading aortic stenosis or planning transcatheter aortic valve replacement (TAVR), where annular sizing, cusp morphology, and coronary ostial height can determine paravalvular leak risk and coronary obstruction. It also supports teaching the mechanism of aortic regurgitation, where cusp prolapse, endocarditis-related perforation, or dilation of the aortic root prevents full coaptation in diastole.
