Description
Arising from the pubic portion of the acetabular rim and the superior pubic ramus, the pubofemoral ligament is shown coursing inferolaterally to blend with the anteroinferior joint capsule of the hip. Its fibers converge with the deep capsular layer near the femoral neck, lying inferior and medial to the more superior anterior capsular thickening often associated with the iliofemoral ligament. Surrounding bony landmarks logically visible include the pubis, ilium, and ischium forming the acetabulum, with the femoral head seated in the socket and the proximal femur extending distally from the joint. Functionally, the pubofemoral ligament is the anterior-inferior checkrein of the hip, limiting excessive abduction and external rotation when the joint approaches extension, and its capsular blending makes it relevant to both stability and capsular mechanics. This relationship matters during anterior and anterolateral hip exposures, where capsulotomy and capsular repair can alter restraint patterns and contribute to postoperative instability if the anteroinferior capsule is inadequately restored.
