Running along the anteroinferior aspect of the male hip joint, the pubofemoral ligament is shown arising from the superior pubic ramus and adjacent pubic portion of the acetabular rim, then blending laterally and inferiorly with the anterior joint capsule near the femoral neck. Medial to its origin lie the pubic symphysis and pubic body, while superiorly the ilium forms the roof of the acetabulum and the lumbar spine and sacrum sit in the midline posterior background. Inferior and posterolateral to the pubis, the ischium completes the obturator foramen margin, and the proximal femur occupies the lateral field as it articulates with the acetabulum. Orientation is clear. Anterior. Functionally, the pubofemoral ligament is the anteroinferior reinforcement of the fibrous capsule, working in concert with the iliofemoral and ischiofemoral ligaments to check excessive hip extension and abduction. This view is the one you want when explaining why anterior hip stability is not a single structure, and why capsular laxity or iatrogenic capsulotomy can change the end range in extension and external rotation after arthroscopy. In anterior approaches to total hip arthroplasty, surgeons also discuss capsular preservation and repair in relation to these anterior thickened bands, since their orientation helps resist anterior translation of the femoral head in extension.
