Description
Anterior to the hip joint, the pubofemoral ligament is shown as a thickened band of the fibrous capsule arising from the pubic body and superior pubic ramus, then sweeping inferolaterally to blend with the anterior capsule near the femoral neck and intertrochanteric region. Superiorly, the iliac crests frame the bony pelvis, while the lumbar vertebrae descend to the sacrum in the midline. Bilateral femoral heads sit lateral to the pubic symphysis within the acetabula, with the capsular fibers spanning from the acetabular rim toward the proximal femur. Clear spatial logic. Clinically, this anterior perspective helps you teach why the pubofemoral ligament tightens in hip extension and abduction, acting as a primary checkrein against excessive abduction and anterior translation of the femoral head. It is also the capsular region incised and repaired during anterior approaches to total hip arthroplasty (Hueter or Smith-Petersen interval), where capsulotomy placement influences postoperative stability and range of motion. Painful restriction in hip extension or abduction after capsular scarring can be understood in terms of these pubic-to-femoral fiber orientations.
