Description
Centered on the anteroinferior hip, the pubofemoral ligament is shown as a capsular thickening extending from the pubic portion of the acetabular rim and adjacent superior pubic ramus toward the femoral neck. Its fibers course laterally and inferiorly, blending with the anterior joint capsule deep to the iliopsoas region and approaching the intertrochanteric line. Superiorly it lies inferior to the iliofemoral ligament, while medially it sits close to the pubic symphysis and the obturator foramen margins on the male pelvis. Clinically, the pubofemoral ligament is the main anterior restraint to hip abduction and contributes to limiting extension when the hip is externally rotated, a relationship that becomes obvious when comparing it to the more superior iliofemoral banding. That matters in anterior hip pain and instability workups where overlengthening of the anterior capsule, whether from repetitive extremes of motion or iatrogenic capsulotomy during hip arthroscopy, can increase abduction and external rotation laxity. A small structure, but a real one. Its pubic origin also explains why capsular injuries and psoas-related impingement can coexist near the anterior acetabular rim. Use this asset for teaching hip joint capsule anatomy in gross anatomy, kinesiology, and orthopedic modules, or to support figures in publications discussing femoroacetabular impingement, capsular management in arthroscopy, and anterior instability patterns. It also reads well in patient-facing materials that need a clear distinction between the iliofemoral and pubofemoral components of the capsule on a male pelvic skeleton.
