Description
Running longitudinally along the posterior thoracic spine, the semispinalis thoracis appears as a deep transversospinalis muscle mass spanning from the transverse processes of the lower thoracic vertebrae to the spinous processes several levels superior. Medial to the costal angles of the ribs, its obliquely oriented fascicles converge toward the midline, lying deep to the superficial erector spinae group and deep to the rhomboid major and minor near the medial border of the scapula. Thoracic spinous processes form the central landmark, with paired ribs projecting laterally and the scapulae resting posterolaterally over the rib cage. Deep back anatomy, made legible. Semispinalis thoracis matters most when you need to teach segmental extension and contralateral rotation of the thoracic spine, and when you want to clarify why thoracic paraspinal pain generators are not always the superficial muscles patients point to. Trigger points and strain patterns in the transversospinalis group can mimic costovertebral or facet-mediated pain, and the close relationship to thoracic zygapophysial joints makes this view useful when discussing medial branch blocks and radiofrequency neurotomy targets in the mid thoracic region. For surgeons and anatomists, it also helps orient layered posterior approaches where the erector spinae are reflected to reach deeper planes.
