Description
Beneath the posterior skin and superficial fascia of an adult male back, the semispinalis thoracis is rendered as a deep transversospinalis column lying medial to the longissimus thoracis and immediately lateral to the thoracic spinous processes. Superiorly it continues toward the cervicothoracic junction, while its fascicles course superomedially from transverse processes to spinous processes across several segments. Overlying landmarks such as trapezius (from nuchal region to mid thorax) and latissimus dorsi (spanning the lower thorax and thoracolumbar fascia) frame the deeper paraspinal mass, with the erector spinae complex forming paired longitudinal ridges on either side of the vertebral column. A posterior perspective makes the midline bony contours, interspinous intervals, and thoracolumbar fascial plane easy to orient.
