Description
Running longitudinally along the posterior thorax, the semispinalis thoracis is presented as a deep transversospinalis muscle spanning multiple thoracic vertebral segments. Its fascicles arise from the transverse processes of lower thoracic vertebrae and course superomedially to insert on the spinous processes of upper thoracic and lower cervical vertebrae, lying medial to the longissimus thoracis and deep to the more superficial erector spinae mass. Thoracic spinous processes sit in the midline, with ribs projecting laterally from their costotransverse and costovertebral articulations; the scapulae are partially included superiorly, draped over the posterolateral rib cage. Segmental layering is clear. Semispinalis thoracis matters because it is a principal posterior stabilizer of the thoracic spine and a frequent source of diagnostic confusion when teaching “back muscles” as a single column. In thoracic pain patterns, deep paraspinal strain and myofascial trigger points can localize near the costotransverse joints, and appreciating the semispinalis fiber direction helps distinguish it from longissimus and spinalis when planning palpation, dry needling, or targeted injection. Surgeons and interventionalists also use the relationship of deep paraspinals to the spinous and transverse processes as a working map for midline posterior approaches, including thoracic instrumentation and procedures near the dorsal rami.
