Description
Beneath a lightly rendered dermal layer on the posterior thorax, the semispinalis thoracis is identified as a deep, longitudinal transversospinalis muscle running superomedially from transverse processes in the upper thoracic region toward spinous processes several vertebral levels above. Superficial to this corridor, the trapezius forms the broad upper back contour, while rhomboid major and minor lie deep to trapezius and course from the thoracic spinous processes to the medial border of the scapula. Laterally, fibers of latissimus dorsi frame the lower portion of the field, and the scapular region includes infraspinatus and teres major along the posterior scapula and proximal humerus. Skin remains intact. Semispinalis thoracis matters when you want to teach or plan around the layered architecture that separates superficial scapulothoracic movers from the deep segmental extensors and rotators that load the thoracic facet joints. In posterior thoracic approaches, the line from spinous process to transverse process helps orient you to the costotransverse region, where dissection and retraction can provoke postoperative paraspinal pain and, less commonly, dorsal rami irritation. For pain medicine teaching, this is the muscle layer you traverse when targeting thoracic medial branch nerves for facet-mediated pain, even though the needle endpoint is on bone rather than in muscle.
