Description
Arising in the superomedial orbit, the supraorbital vein courses inferiorly and medially along the forehead, then approaches the medial canthus where it joins the supratrochlear vein to form the angular vein at the root of the nose. The vessel lies superficial to the frontal bone and supraorbital margin, with the supraorbital nerve and artery traveling in close proximity as they emerge near the supraorbital notch or foramen. Deep to this superficial venous channel, the bony orbit, nasal bones, maxilla, and anterior cranial base provide rigid landmarks, while cervical vertebrae anchor the inferior field of view. Venous tributaries in the periorbital plexus communicate across the midline and toward deeper facial and ophthalmic pathways. That confluence at the medial orbital rim matters clinically because the angular vein connects with the superior ophthalmic vein, creating a valveless route between superficial facial venous drainage and the cavernous sinus. Infection in the so called danger triangle of the face, or septic thrombophlebitis after manipulation of a furuncle near the nose, can spread retrograde through these channels and present with orbital cellulitis, proptosis, and cranial neuropathies. Surgeons and injectors also track this anatomy when planning upper blepharoplasty, endoscopic brow lift portals, or cosmetic filler placement, where inadvertent intravascular injection near the supratrochlear and supraorbital vessels can have immediate orbital consequences.
