Description
Sweeping across the superior margin of the upper eyelid, the superior palpebral venous arcade forms a curved venous loop within the tarsal plate and pretarsal tissues, lying anterior to the orbital septum and superficial to the levator palpebrae superioris aponeurosis. Small venules converge medially and laterally toward the canthi, where they communicate with the angular vein medially and the lateral palpebral and superficial temporal venous channels laterally, while remaining closely related to the orbicularis oculi in the anterior lamella. An expansive facial context is included, with the bony orbit and adjacent craniofacial landmarks framing the lid vasculature. Fine connections to the periocular venous plexus are implied. Upper-lid venous anatomy matters because seemingly minor venules become major landmarks once you are working in the thin planes of blepharoplasty, ptosis repair, or eyelid laceration closure, where small tears can obscure the tarsal border and prolong operative time. Medial canthal venous connections are also clinically charged: venous drainage from the palpebral region can communicate with the angular and ophthalmic veins, a route implicated in the spread of infection from the so-called danger triangle of the face toward the cavernous sinus. This is not abstract anatomy. It is surgical reality.
