Description
Arising from tributaries at the medial canthus, the superior ophthalmic vein courses posteriorly along the roof of the orbit, tracking superior to the extraocular muscle cone and in close relationship to the frontal bone and orbital plate. Medially, its early channels communicate with the angular vein and the dorsal nasal vein, then the vessel sweeps toward the orbital apex near the optic canal and superior orbital fissure. From an anterior perspective, the bony orbital rim and adjacent craniofacial skeleton frame the venous pathway, with accompanying arterial branches rendered in red to clarify the periocular vascular neighborhood. Blue venous tributaries form a plexiform pattern along the superior orbit before consolidating into the main trunk. Clinically, the superior ophthalmic vein is the dominant venous egress of the orbit and a key conduit between facial venous drainage and the cavernous sinus via the superior orbital fissure. This anatomic continuity explains why infections from the “danger triangle” of the face can propagate to cavernous sinus thrombosis, and why orbital cellulitis may present with chemosis and proptosis when venous outflow becomes impaired. Dilatation of the superior ophthalmic vein on CT or MR angiography also raises concern for a carotid cavernous fistula, where arterialization of cavernous sinus flow reverses normal venous dynamics and distends this vessel. Small details matter here. The absence of valves in much of this venous network.
