Description
Seen in lateral profile, the inferior palpebral venous arcade forms a fine blue venous loop within the lower eyelid, coursing just anterior to the orbital septum and superficial to the tarsal plate before draining toward the medial and lateral canthal regions. Posterior and superior to the arcade lie the bony margins of the orbit, with the zygomatic bone laterally and the maxilla inferiorly, while the mandible and dentition sit further inferior and posterior in the same plane. Small venules interconnect the arcade with adjacent facial veins, tracking over the malar region and toward the angular and superficial temporal venous pathways. Bone provides the fixed frame. Surgeons and injectors care about this vessel pattern because the eyelid and periorbital venous network communicates with the angular vein and, through the ophthalmic veins, with the cavernous sinus, so infections or septic thrombophlebitis can propagate from seemingly minor lower lid lesions. This lateral view also supports teaching the distinction between superficial periocular veins and deeper orbital structures when planning lower blepharoplasty, transconjunctival approaches, or managing postoperative ecchymosis and hematoma.
