Description
Arising from the facial nerve (cranial nerve VII) after it exits the stylomastoid foramen and traverses the parotid region, the zygomatic branch courses anterosuperiorly across the zygomatic arch toward the lateral orbital rim. Fine rami spread superficially over the temporal fossa and malar region, running in the same plane as the SMAS and investing fascia, and passing adjacent to the zygomaticus major and minor as they sweep superomedially toward the modiolus. Orbicularis oculi encircles the palpebral fissure anteriorly, while the masseter sits posterior and inferior to the zygomatic body, providing a clear muscular landmark for the nerve’s relationship to the cheek. Small arterial and venous channels accompany the nerve across the face. Delicate anatomy. For facial reanimation and cosmetic surgery, this is the branch you worry about when working near the zygomatic arch and lateral canthus. Injury to the zygomatic branch can weaken orbicularis oculi closure, predisposing to exposure keratopathy, and it can blunt the action of the zygomaticus complex, flattening the smile on the affected side, a pattern commonly evaluated in Bell palsy and post-parotidectomy facial weakness. This perspective also supports teaching the functional map of CN VII by linking a named branch to its target muscles rather than treating the facial nerve as an abstract plexus.
