Running along the dorsum of the male forefoot, the dorsal digital veins course longitudinally on the medial and lateral margins of each toe and interconnect across the proximal phalanges as a superficial venous network. Tributaries converge proximally toward the dorsal metatarsal veins over the metatarsal shafts, with perforating channels passing plantarly through the intermetatarsal spaces to communicate with the plantar digital veins. Distally, small venules arise from the nail bed and dorsal toe skin, while the plantar contributors originate deep to the phalanges and emerge to join the dorsal system. Superficial fascia remains the immediate bed, with extensor tendons lying deep and slightly medial or lateral depending on the toe. A dorsal perspective on plantar digital venous origin is more than an anatomic curiosity because it explains why distal toe swelling and cyanosis can persist when superficial dorsal channels thrombose or are disrupted, even if larger proximal veins remain patent.
