Description
Medial pterygoid plate of the sphenoid appears as a thin, vertically oriented lamina descending from the pterygoid process, seen from the anterior aspect within the posterior nasal region of the skull. Superiorly it blends with the root of the pterygoid process at the sphenoid body, while inferiorly it ends in the pterygoid hamulus, a hooklike projection that curves posterolaterally. Along its medial margin, the plate helps define the lateral boundary of the choanal aperture and lies adjacent to midline elements such as the vomer and rostral sphenoid, with the pterygoid fossa situated lateral to it between the medial and lateral pterygoid plates. The hamulus stands out. Clinically, the hamulus is a reliable landmark because the tendon of tensor veli palatini turns around it before spreading into the palatine aponeurosis, so subtle variation in hamular length or angulation can relate to soft palate mechanics, otitis media with effusion, and postoperative orientation during cleft palate repair. Pain localized to the posteromedial maxillary region can also reflect hamular bursitis, and this anterior perspective supports correlation with dental and ENT exam findings by keeping the hook, plate margins, and choanal relationship in a single frame. Fracture lines from midface trauma (including Le Fort patterns) can involve the pterygoid processes, and appreciating the medial plate’s thin profile helps explain why this region is susceptible. Use this illustration in gross anatomy teaching of the cranial base, in dentistry and maxillofacial surgery texts discussing pterygoid process fractures, and in otolaryngology materials that link the pterygoid hamulus to tensor veli palatini function and Eustachian tube physiology.
