Description
Medial pterygoid plate of the sphenoid descends from the body of the sphenoid as a thin lamina, seen from an inferior viewpoint along the posterior aspect of the nasal cavity. Its free inferior margin narrows to the pterygoid hamulus, the hook-like process projecting posterolaterally at the distal end of the plate. Medially, the plate forms part of the lateral boundary of the choanal region and contributes to the pterygoid fossa when considered with the adjacent pterygoid elements, while laterally it lies in the anatomic neighborhood of the lateral pterygoid plate and the infratemporal surface of the skull base. Bone contours are crisp. Clinical relevance centers on the hamulus and the soft palate mechanics it supports, because the tendon of tensor veli palatini turns around the pterygoid hamulus as a pulley before expanding into the palatine aponeurosis, and irritation at this interface is discussed in hamular bursitis and some cases of orofacial pain. In midface trauma, fractures involving the pterygoid plates are a defining feature of Le Fort patterns, so a clean inferior perspective helps orient learners to where pterygoid disruption is assessed on CT and how the pterygoid region relates to the posterior maxilla and palate.
