Description

Arising from the ileocolic artery within the mesoappendix, the appendicular artery tracks distally along the medial border of the vermiform appendix toward its tip, giving small branches that pierce the muscular wall. In this frontal (anterior) view, the cecum sits superior and lateral to the terminal ileum, with the appendix projecting inferiorly from the posteromedial cecal wall near the convergence of the taeniae coli. Accompanying veins mirror the arterial course in blue, while yellow autonomic nerve fibers form a periarterial plexus that follows the vessels toward the appendiceal base and cecum. This perspective matters because the appendicular artery is functionally an end-artery for much of the appendix, so kinking or thrombosis during appendiceal inflammation can accelerate ischemia and gangrene at the distal third. It also maps directly onto operative strategy: during appendectomy, safe division of the mesoappendix close to the appendix protects the ileocolic branches supplying the cecum and minimizes bleeding from the appendicular pedicle. Small vessel caliber and variable branching explain why hemostasis can be deceptively challenging when the mesoappendix is edematous.
