Description

Arising most commonly from the ileocolic artery, the appendicular artery runs in the free margin of the mesoappendix to reach the vermiform appendix at the inferomedial aspect of the caecum. An anterior view places the caecum and proximal ascending colon in the right lower quadrant, with terminal ileum approaching medially and the taeniae coli converging toward the appendiceal base. The vessel’s characteristically slender, tortuous course is traced distally along the appendix toward its tip. Small branches to the appendiceal wall and adjacent mesentery are typically suggested by fine arborization. Surgical relevance is straightforward. Because the appendicular artery is often an end artery with limited collateral supply, thrombosis or kinking during acute appendicitis can accelerate mural ischemia and gangrene, and incomplete ligation during appendectomy can lead to persistent bleeding into the mesoappendix. This view also helps orient the mesoappendix as the structure you divide and secure when mobilizing the appendix, and it supports discussion of anatomic variation, including accessory appendicular arteries arising from posterior cecal branches or the right colic system.
