Description
Seen from the left lateral aspect, the posterior cranial fossa is opened to emphasize the confluence of sinuses (confluens sinuum) at the internal occipital protuberance, where the superior sagittal sinus meets the straight sinus and drains into the paired transverse sinuses along the attached margin of the tentorium cerebelli. The occipital sinus may be traced in the midline along the internal occipital crest toward the foramen magnum, sitting posterior to the cerebellar vermis and inferior to the splenium region and posterior cerebral hemispheres. Superficial cortical veins and bridging veins course from the cerebral convexity toward the superior sagittal sinus, while deeper venous channels converge toward the great cerebral vein and straight sinus. Color coding separates arterial supply on the cerebral surface from the dural venous pathways and adjacent cranial nerves at the brainstem. Understanding this junction matters in dural venous sinus thrombosis, where occlusion at or near the confluence can redirect flow, raise intracranial venous pressure, and produce posterior headache, papilledema, or hemorrhagic venous infarction. It is also a key landmark during posterior fossa and occipital craniotomy planning, because the transverse sinus location and dominance vary and inadvertent sinus injury can cause brisk bleeding or air embolism. A surgical no-go zone. Use this lateral perspective when teaching cranial meningeal anatomy in neuroanatomy or head and neck courses, or when illustrating venous drainage pathways for radiology and neurosurgery texts that correlate MRV/CTV findings with operative landmarks.
