Viewed laterally in the lumbar spine of an adult male, the interspinales appear as short, fleshy bundles spanning between adjacent spinous processes, lying posterior to the vertebral canal and deep to the larger erector spinae mass. Each slip runs nearly vertical, with small tendinous attachments blending into the periosteum of the spinous tips and the interspinous ligament. Nearby bony landmarks include the laminae and zygapophyseal (facet) joints, while the intervertebral discs sit anterior to the vertebral arches as pale, cushioning pads between vertebral bodies. Small muscles. Tight spacing. Because the interspinales contribute to fine segmental extension and proprioceptive control, this lateral perspective helps teach why posterior midline pain can persist even when the major paraspinals look intact on gross inspection.
