Description
Seen from a lateral perspective, the quadratus plantae (flexor accessorius) lies deep in the plantar compartment, arising from the calcaneus and coursing anteriorly toward the flexor digitorum longus tendon. Its muscle belly sits inferior to the calcaneus and posterior to the metatarsal shafts, with the long plantar ligament and plantar aponeurosis positioned more superficially. Medially it relates to the neurovascular plane of the medial plantar nerve and artery, while laterally it approaches the lateral plantar nerve and artery as these structures sweep forward between muscle layers. Quadratus plantae matters because it modifies the line of pull of flexor digitorum longus, helping direct toe flexion along the longitudinal axis of the foot rather than obliquely from the leg. That small correction becomes clinically tangible in the context of plantar heel pain, post traumatic calcaneal deformity, and surgical dissection for tarsal tunnel or medial plantar nerve decompression, where the deep plantar compartment can be confusing without a clear mental map. A hard working landmark. It also provides orientation when tracing the lateral plantar neurovascular bundle during approaches to the midfoot and plantar fascia procedures.
