Description
Medial soft tissues of the plantar foot are rendered with the skin as a transparent envelope, allowing the quadratus plantae (flexor accessorius) to be appreciated deep to the plantar aponeurosis in the second muscular layer. Its medial and lateral heads arise from the plantar surface of the calcaneus and converge anteriorly to insert into the posterolateral margin of the flexor digitorum longus tendon, just distal to the calcaneal tuberosity and proximal to the midfoot. Superior to the muscle sit the calcaneus and talus with the tibia continuing proximally, while anteriorly the navicular, cuneiforms, cuboid, metatarsals, and phalanges provide bony context for the plantar tendons coursing toward the toes. Joint cartilage is indicated along the tibiotalar and midfoot articulations. Orientation is clear. Quadratus plantae matters because it redirects the oblique line of pull of flexor digitorum longus into a more longitudinal vector, which stabilizes toe flexion during late stance and helps limit unwanted varus deviation of the lesser digits. Clinically, its relationship to the medial calcaneal tubercle and the course of the lateral plantar nerve (including the first branch, Baxter nerve) makes it a relevant landmark when discussing plantar heel pain, nerve entrapment, and operative or injection approaches that aim to avoid iatrogenic injury. The close proximity to the long plantar tendons also explains why altered mechanics after calcaneal fracture, plantar fasciotomy, or chronic pes planus can change digital flexion patterns and contribute to clawing. Use this illustration for teaching the layered anatomy of the sole in gross anatomy, podiatry, and orthopaedic foot and ankle courses, or as a figure in a review on plantar heel pain, accessory muscles of the foot, and tendon biomechanics.
