Description
Arising from the plantar aspect of the calcaneus, the quadratus plantae (flexor accessorius) is shown deep to the plantar aponeurosis and superficial to the long flexor tendons as it courses anteriorly toward the midfoot. Medially, fibers typically take origin from the medial process of the calcaneal tuberosity, while a lateral head often reflects from the lateral plantar calcaneus and adjacent long plantar ligament, converging into a flattened, quadrangular belly. Distally, the muscle inserts into the posterolateral margin of the flexor digitorum longus tendon, just proximal to its division toward the lateral four toes, placing it inferior to the tarsals and plantar to the metatarsals and phalanges in this plantar-oriented presentation. Bones of the forefoot and hindfoot provide orientation, with the calcaneus posterior, metatarsals anterior, and phalanges most distal. Quadratus plantae matters because it corrects the oblique pull of flexor digitorum longus, redirecting force into a more longitudinal line for efficient flexion at the metatarsophalangeal and interphalangeal joints. Small muscle, big effect. In posterior tibial tendon dysfunction and acquired flatfoot, altered hindfoot alignment changes the mechanics of the long flexor tendons and can increase strain along the medial plantar structures, a context where understanding this deep layer helps explain toe clawing and compensatory digital flexion. The same anatomy becomes relevant during plantar surgical approaches that work around the calcaneus and deep flexor compartment, where the lateral plantar nerve and vessels run in close relationship to the quadratus plantae and adjacent flexor digitorum brevis.
