Description

Exposed on the plantar aspect of the male foot, the quadratus plantae lies deep to the plantar aponeurosis and flexor digitorum brevis, occupying the central compartment between the medial and lateral plantar muscle groups. Its medial head arises from the medial surface of the calcaneus, while the lateral head takes origin from the lateral plantar surface of the calcaneus and the long plantar ligament, converging anteriorly to join the tendon of flexor digitorum longus (FDL). Distally, the FDL tendons continue toward the lateral four toes, with the quadratus plantae positioned posterior to the metatarsals and inferior to the calcaneus, correcting the oblique pull of FDL in the sagittal plane. For teaching function, this is the cleanest way to understand why quadratus plantae exists at all: the FDL tendon enters the sole from the medial side, and without this muscle the line of pull would tend to draw the toes medially during flexion. It matters in surgery and pain practice, too. The muscle sits near the course of the lateral plantar nerve and vessels, and it is a frequent landmark when tracing plantar neural entrapment patterns, differentiating Baxter nerve (first branch of the lateral plantar nerve) irritation from more proximal tarsal tunnel pathology. Use this plate in gross anatomy labs and podiatric anatomy courses when introducing the layered dissection of the sole, or in operative atlases covering plantar approaches to the calcaneus, tendon transfer planning, and selective denervation for chronic heel pain.
