Description
Plantar dissection exposes the quadratus plantae (flexor accessorius) deep in the central sole, spanning from the plantar surface of the calcaneus toward the tendons of flexor digitorum longus. A medial head arises from the medial concavity of the calcaneus and adjacent plantar aponeurosis, while a lateral head originates from the lateral border of the calcaneus and the long plantar ligament, the two portions converging anteriorly. Just superficial sit the flexor digitorum brevis and plantar aponeurosis, while the quadratus plantae lies posterior to the lumbricals and tends to blend with the posterolateral aspect of the flexor digitorum longus tendons as they course toward the toes. Orientation is everything. Functionally, quadratus plantae modifies the line of pull of flexor digitorum longus so toe flexion occurs with less oblique deviation, a point that becomes obvious when you track the muscle fibers from their posterior calcaneal origins to the distal tendons in the midfoot. The structure is also a useful landmark when teaching the layered plantar foot and the course of the lateral plantar nerve and artery, which pass in close relation to the deeper flexor compartment and can be encountered during plantar approaches for foreign body removal or fibromatosis excision. Chronic heel pain discussions often focus on plantar fasciitis, but tension and overload in the deep posterior sole, including quadratus plantae, can contribute to persistent symptoms and altered toe flexion mechanics.
